Certificate of Insurance

If your meeting location requires you to obtain a certificate of insurance (COI), please send an email to waivicechair@foodaddicts.org and include the following information:

1. Day of week and time of the meeting

2. Start date (for a new meeting)

3. Name of meeting location/facility 

4. Address of meeting location 

5. Meeting contact person (first and last name) and phone number
